Organizations that already manage complex cases, from clinical case management and companion services to concierge navigation firms and private-client offices, retain me to head the cases that need senior judgment. I lead the case as a whole, and your team delivers the day-to-day work under that direction.
Organizations typically call when a case has become more complex, more consequential, or more contested than the assigned team can carry on its own.
The clinical picture, the family, or the number of providers involved has become more than the assigned staff can manage alone.
Several people are speaking with the family, and the family needs one senior advisor to lead the conversation.
Providers disagree about level of care or next steps, and someone has to weigh the recommendations and set a course.
Risk is rising, boundaries are slipping, or the client is disengaging, and the assignment needs senior direction.
A discharge, step-down, or move between programs needs a plan and someone accountable for seeing it through.
Discretion, reputation, and the number of advisors and staff involved call for senior leadership of the case.
I head the case. The organization brings its staff and the day-to-day delivery, and I hold the direction of the whole matter, which means deciding what the case is working toward, how each person involved contributes to it, and what has to change when circumstances do.
In practice, that begins with an assessment of the case and a plan that sets the priorities and the roles of everyone involved. From there I lead the case managers or companions carrying out that plan, reviewing the work with them and adjusting course as the case develops, and I serve as the senior point of contact for the family, the treating providers, and the other advisors, so that the case moves under one direction rather than several.
When the consequential decisions arrive, whether they concern level of care, a transition, the family's boundaries, or a case that is escalating, I lead them, drawing on what the organization's team and the treating providers are seeing.
The division of responsibility is set for each case after I assess it. Depending on what the case requires, I may direct the delivery of the plan closely, or lead the strategy and the family relationship while your organization supervises its own staff, and in many cases it is a combination of the two. The arrangement is written into the engagement agreement before the work begins, so that your team, the family, and I all know who decides what.
Your organization remains responsible for its staff, its services, and its own obligations to the client, and treatment responsibility remains with licensed treating professionals. My role is advisory leadership of the case. It is not clinical treatment, and it does not replace emergency services.
When an organization retains me, my fee for the advisory work on the case is the only compensation involved. I accept no referral fees, I do not direct clients to any organization in exchange for anything, and the family is told at the outset that the organization has retained me.
Organizations engage me per case, as a Defined Engagement for a focused assignment or Ongoing Advisory for the life of a case, or through a standing partnership for case reviews and escalations across several clients. Scope and terms are set in writing, and representative scenarios appear in The Work in Action.
To discuss a case or a standing partnership, call or write directly. A first conversation about a specific client can take place in outline, without identifying details.
